O2ptima Liability Release Waiver

A signed “Statement of Understanding” and “Liability Release” is required before starting an O2ptima CCR course. An updated “Statement of Understanding” and “Liability Release” is required when servicing or upgrading your O2ptima CCR, and when completing additional training.

Please read and check each of the following statements carefully. By checking the boxes, you are certifying that you understand each. *

Closed circuit rebreathers like the Optima make no bubbles and the ability to take a breath does not ensure survival. I must analyze my gases and make sure my scrubber duration is sufficient for the duration of the dive.

During the dive, I must always know my PPO2 and monitor it for safe PPO2 levels. Failure to know my PPO2 can result in serious injury or death.

The Optima requires regular maintenance and the recommended schedule is every 12 months.

Oxygen sensors should not be used for more than 12 months and not beyond the expiration date regardless of installation date.

I must always have adequate bailout gas for each dive. This gas should be with you at all times during a dive.

Rebreather diving is an inherently dangerous activity. I will discuss my diving activities with people close to me so they understand the personal risks and responsibilities I assume when diving with rebreathers including the O2ptima.

There can be increased health risks when diving with rebreathers. I will make sure I am physically, medically and mentally fit to undertake this activity prior to diving.

I will use and follow the manufacturer's checklists to build and maintain my unit and for pre-dive checks.

I must keep my Optima clean in order to avoid certain increased health risks.

I may only use the Optima under the supervision of a qualified instructor until l successfully complete an approved Optima training course.

I will complete any additional training required following any upgrades to my Optima.

I will not loan my Optima to someone who is not trained and certified to use it. I am solely responsible for the use of my personal gear.

I will not dive beyond my training and/or my personal comfort.

No one can breathe for me. Maintenance, safety checks and appropriate gear selection is my responsibility.
